Risk Assessment Matrix

The aim of this risk assessment is to determine priorities for action - not produce a scientific thesis. You will decide later what practical steps to take to control.

Task 1. Decide the range of risks you are going to compare (eg physical, chemical, biological, climatic, geographic) and enter in box below

Task 2 Type your chosen risk in the coloured box in the grid eg. cut, slip, poisoning.
Choose the appropriate box by selecting the severity of the risk weighed against the likelihood of it happening.

Task 3. Explain in the right hand side text box why you consider this risk to be the severity and likelihood you have chosen.

Task 4. When complete, print this page for your portfolio before clicking the submit button.
Your answers will be sent directly to your assessor.
